(This form must be received in Student Affairs 2 weeks prior to trip. Each individual must sign a liability waiver prior to departure. Expenses for trip will not be approved until travel request has been approved. General Counsel will also review all travel requests. Special instructions may be communicated upon General Counsel's review. Clubs and organizatons or other student groups are expected to follow these instructions closely.)
